Assertive Community Treatment
Overview
- Individual-centered, recovery-oriented, and a highly intensive community based service for individuals who have serious and persistent mental illness.
- Individuals that qualify for this service have been unsuccessfully treated in the traditional mental health service system because of their high level of mental health acuity.
- Typically, individuals have had:
- 10 or more psychiatric hospital admissions,
- 3 or more psychiatric hospital admissions in the past year
- Frequent visits to the Emergency Room for psychiatric reasons,
- Frequent contact with law enforcement, and/or
- Chronic homelessness due to mental illness.
- Each ACT Team serves 75-100 individuals
